The conflict mentioned in the article isn't necessarily a conflict. If Kamal was in a modern-day setting, according to a Wikipedia article (http://en.wikipedia.org/wiki/Physician) on current American medical training, he could be a medical resident. He wouldn't necessarily be a permanent resident yet (and hence doesn't consider himself "done with school and all that") but he has technically finished med school. There's another possible interpretation for "residency". An immigrant to the US must apply for a residency permit, either for a term or a Lawful Permanent Residency (http://uscis.gov/graphics/services/residency/).
